All you need to know about Care Plans
What is a Care Plan?
A Care plan, also known as a GP Management Plan, is a written plan of goals to manage chronic conditions (present for at least 6 months) developed by your GP and Practice Nurse.
These plans involve Allied Health professionals and are reviewed every 3 to 6 months to aid in progress and goals being achieved and assess whether any changes are to be made.
Care plans are available to our regular patients with ongoing medical conditions lasting more than 6 months.
Medical conditions eligible for Care Plans include conditions such as:
Diabetes
Cancer
Heart Disease
Asthma
Arthritis
Osteoporosis
Or any other medical condition that has been present for more than 6 months
Once a Care Plan has been created you may be eligible to see any allied health professional for 5 visits per year under Medicare.
Allied health professionals can include:
Chiropractor
Diabetes Educator
Dietitian
Mental Health Worker
Physiotherapist
Podiatrist
Occupational Therapist
Care Plan Reviews
Once a plan is in place, it should be regularly reviewed by your GP. This is an important part of the plan cycle, where you with your GP and practice nurse check that your goals are been met and agree on any changes that you may require.
A Care Plan Review is usually under taken every 3 to 6 months.
Assessments conducted in clinic are:
45 - 49 year old
75 years and over
Aboriginals and Torres Strait Islanders
Permanent residence of age care facilities
People with an intellectual disability
Refugees and other humanitarian entrants
Health Assessments
The 45 - 49 year old health check is a one off check up aimed at assessing your risk of developing chronic disease in the future. The Health Check is an opportunity to reflect on your lifestyle, medical and family history and to then assess your risk of developing chronic disease.
A health assessment of an older person is an in-depth annual assessment of a patient aged 75 years and over. It provides a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and/or quality of life.
The assessment is conducted by our experienced Nurses for 45 minutes - 1 hour and the purpose of these assessments are to provide services to maintain your independence within your own home. You can expect discussions surrounding potential care plans, home help, medication reviews and immunisations.
